Athlete Physical/Pre-Participation Exam

All World Cup and World Championship team members are required to have a yearly physical/pre-participation exam prior to traveling with the team. This form must be signed/stamped by a Doctor of Medicine (MD) or Doctor of Osteopathy (DO); no other medical providers will be accepted (ATC, DC, PT, etc.).  If you are a minor, please use your Pediatrician’s physical form.
